What is a palatal expander?

 

Rapid Palatal Expander

A palatal expander, also known as a rapid palatal expander, rapid maxillary expansion appliance, palate expander or orthodontic expander, is used to widen the upper jaw so that the bottom and upper teeth will fit together better. It is thought this can only be done when the patient is still growing, unless surgery is used to separate the two halves of the palate, however there is evidence to the contrary. It is most often followed by braces to straighten out all the teeth now that room has been created.

The expansion process usually results in a large gap between the patient’s top front teeth, although this does not always happen. This gap is closed naturally and the teeth may overlap which leads to braces being needed. Sometimes with expanders, the patient has to turn the expansion screw themselves to tighten up the expander. For expansion that is not managed by the patient on the and lower jaw, a bionator may be a more suitable alternative.

Although it may vary from person to person, most usually feel slight pressure on their teeth. As the patient turns the expansion screw using the key, a space develops between the front two teeth. Some may notice a larger space while others do not notice a space at all. It usually takes several days to adjust to eating and speaking after first receiving the rapid palatal expander. Patients may experience pain and headaches while wearing palatal expanders and when the screw is turned. They may also feel a sore on their tongue from contact with the expander’s metal bars.

Patients who have expanders may experience extra saliva and lisps (pronouncing the letter S as a T sound) or a slight ‘hissing’ S sound.

When the palatal expander is first removed, patients usually report that their mouth feels extremely big and spacious.

Once braces are removed, we bond a lingual wire/s to the inside of the front teeth to hold these teeth in place as we know your front teeth have a potential to relapse (go back to the way they were before treatment).
THE WIRE MUST STAY IN PLACE FOR AS LONG AS POSSIBLE FOR YOUR FRONT TEETH TO REMAIN STRAIGHT!
Once the wire/s are fitted, you may find that your tongue will play with the wire and that your speech is affected, however, these feelings will recede after a short period of time.

If you are fitted with a PFLR, you will need to:

  1. Eat normally, use your teeth normally, do not tear at crunchy foods or crunch ice-cubes, etc. with your teeth.
  2. Keep the wire/s clean by using a toothbrush and dental floss.
  3. Attend regular dental reviews and have recommended treatments as required by your dentist (usually every 6 or 12 months).

The head gear  has  helped us to  effectively control the growth of  maxillary  and mandibular bones during the orthodontic treatment.

Headgear  is a general name for a type of appliance that creates  special forces for guiding the growth of your face and jaws.We also use headgears to move your teeth into better position or to prevent them from moving when they are not suppose to. Headgear is typically used in growing patients to correct overbites by holding back the growth of the upper jaw, allowing the lower jaw to catch up. The headgear is attached to the braces via metal hooks or a facebow and is anchored from the back of the head or neck with straps or a head-cap. In some situations both are used. Elastic bands are then used to apply pressure to the bow or hooks. Its purpose is to slow-down or stop the upper jaw from growing, hence preventing or correcting an overjet.

 

A typical scenario is a case where some teeth are extracted, and front teeth are being retracted (pulled backward). When extraction spaces are being closed, the teeth behind the extraction space slide forward if not held in-place by headgear. In some situations, to maintain the bite, the orthodontist will need prescribe a headgear at the end of the treatment for another six months period.

The headgear serves to hold them back (maintain anchorage). Orthodontists will want those patients to wear the headgear as much as they can once fitted. Realistically this might only be 12 hours per day, with catch over weekends, but the more the better; even 22 hours may be required. This depends on how fast the patient is growing.

 

Soreness of teeth when chewing, or when the teeth touch, is typical. Adults usually feel the soreness 12 to 24 hours later, but younger patients tend to react sooner, (e.g., 2 to 6 hours).

 

To get the best results from your head gear , it is advised to follow these simple guidelines :

 

·     Make sure to wear your headgear as directed. It only works when you wear it!

·     Always closely follow our instructions for wearing your headgear. If you do, you’ll finish this part of your treatment as quickly as possible and with the best results

·     Always be careful and remove your headgear as you were taught. If you remove the headgear carelessly, the part that fits in your mouth and attaches to your teeth could injure your cheeks, lips, face or even your eyes.

·     Be sure to remove your headgear before you begin running or playing sports, including rough-and-tumble games. Accidents can happen even when you’re just having fun.

·     Be careful to prevent anyone from grabbing or pulling on your headgear. Brothers, sisters or friends who don’t wear headgear may not understand the dangers involved, even in play.

·     Take your headgear with you to every appointment.

·     We will give you instructions on the care and cleaning of your headgear. To keep the appliance working

Clear Braces are a clear alternative to Metal Braces

Here is what she has to share about her experience, “My experience with clear braces has been quite extraordinary ! I knew for a very long time that I’d be needing braces, but I continued to put it off because I just didnt like the thought of having metal in my mouth. When Dr. Pannu told me about clear braces, I was a little confused and very curious. I had never seen anyone with clear braces; everyone I knew had metal braces. At first, I thought how they’d look and how obvious they would be, but when Dr. Pannu showed me a model with the clear braces, I realized that they’re very subtle. I still, almost two years later, have people say, “What? You have braces?” when I mention anything about them. But other than the appearance, the impact has been great, as well. As you can see from the pictures, my teeth in the beginning were very crooked. Some people, including myself, thought that only a miracle would be able to fix them, but i still took the chance with the clear braces, and today, everyone’s just waiting for the braces to come off. Of course, having braces isn’t exactly easy. It’s necessary to be cautious of what you’re eating, and to stay away from hard foods or things like caramel or taffy. Thumbsucking damage treated by Preventive Orthodontics

Abby Gill had a anterior open bite due to thumb sucking. The bite was corrected by preventive orthodontic treatment at Pannu Dental Care.

Children suck on things because sucking is one of a baby’s natural reflexes and as infants get older it serves many purposes. It may make them feel secure and happy and helps them learn about their world to suck on their thumbs, fingers, pacifiers or other objects. Young children may also suck to soothe themselves. Since thumbsucking is relaxing, it may help induce sleep.

After the permanent teeth come in, sucking may cause problems with the proper growth of the mouth and alignment of the teeth. It can also cause changes in the roof of the mouth. The intensity of the sucking is a factor that determines whether or not dental problems may result. If children rest their thumbs passively in their mouths, they are less likely to have difficulty than those who vigorously suck their thumbs. Some aggressive thumbsuckers may cause problems with their baby (primary) teeth.
