Once braces are removed, we bond a lingual wire/s to the inside of the front teeth to hold these teeth in place as we know your front teeth have a potential to relapse (go back to the way they were before treatment).

THE WIRE MUST STAY IN PLACE FOR AS LONG AS POSSIBLE FOR YOUR FRONT TEETH TO REMAIN STRAIGHT!

Once the wire/s are fitted, you may find that your tongue will play with the wire and that your speech is affected, however, these feelings will recede after a short period of time.

If you are fitted with a PFLR, you will need to:

1. Eat normally, use your teeth normally, do not tear at crunchy foods or crunch ice-cubes, etc. with your teeth.
2. Keep the wire/s clean by using a toothbrush and dental floss.
3. Attend regular dental reviews and have recommended treatments as required by your dentist (usually every 6 or 12 months).

What is invisalign case refinement?

As you can see from the 2 above pictures, the after picture still shows there is still mild crowding; therefore, refinement is necessary to fine tune and perform finishing movements to complete the treatment. Getting a refinement done on your Invisalign treatment can be simple. This means that Invisalign will make more trays for you, until you reach the look you want. Refinement records are the same as the records taken to begin your invisalign treatment. A set of accurate molds are taken and these are sent to invisalign for a new clincheck video to be set up. This video will determine the number of aligners and therefore the refinement treatment time. You wear the refinement aligners in exactly the same way as you did previously. The dog ate my homework… err my Invisalign aligners!

“Dr. Pannu! I don’t mean to be cliché but I caught my dog masticating on them the other day and it was too late for me to save it! It is the 16^th set of the series. I know I will need to eventually use it but can I just skip to 17^th from the 15^th? If not, how much will you charge me for this?”

“It happens once in a while that a patient might have lost an aligner, but this is the first time I’ve ever heard of them being chewed on by a dog. I would recommend you to wear number 17, it may feel tight for a day or so, but you will be eventually able to fit in it. In your case we need another round of Invisalign sets anyways, which is called case refinement. This procedure takes care of any more discs that need to be addressed because you are so close to the enhancement. However, if the last aligner #17 doesn’t fit, please let us know and we shall take new impressions. There is no charge for this. Thanks for sharing the pictures of your dog. I was watching The Late Night show with Conan O’Brien the other night and was immensely entertained with William Shatner’s poetic take-off on Sarah Palin’s speech. I had been an avid follower of “Captain Kirk’s” voyages since I was a kid and have a great admiration for this guy who was able to evolve as an actor through the years. And although Shatner’s career hasn’t always been that hot & suffered typecasting badly after Star Trek and his one attempt at a TV Cop series, he really reinvented himself with Boston Legal. So naturally, being interviewed by his team at the “Heartbeat of America” show was not just a pleasure but an honor as well. If the aligners don’t fit…

I tried to put in my second upper Invisalign tray tonight and I can’t get it in.  I’m starting to wonder if there’s something not right with the aligner itself. I am going to leave my first one in over night. Please let me know what to do when you get a chance.

Dear C,

Due to your severe crowding, this is expected every time you change the upper aligners for the first few months. Manufacturing defects are extremely rare in Invisalign aligners.

I recommend using a mirror to guide you in the insertion of your aligners since there’s a great chance that you may be pressing the back portion of the aligners in the wrong areas.

Try to insert from different possible angles by putting it in the front teeth (anteriors) first. Look in the mirror if the back edge of the aligner is dangling towards the palate or the cheeks, and then push the back part of the aligner towards the proper seating.

If that doesn’t work still, insert the left back first, and keep on pressing it towards the anterior (front teeth), making the full arch of the aligner seat onto the full arch of the teeth… If that doesn’t work, do the opposite and start from the right back part.

Different arches have different angles for easy insertion/removal. Hence this process is not the same for all the patients. But believe me; it gets really easy after you get the hang of doing it.

If you have tried everything possible but still have difficulties then please come to the office and I shall help you put it on. After using some series of aligners, this process will become a piece of cake for you.

Can I shorten the time frame of my Invisalign use?

“Hello Dr. Pannu! I am currently on my 2nd set of Invisalign aligners.  I feel tight only on the 1st day or so after I start using a new set, and after that, I don’t feel a lot of pull.  I am wondering if instead of wearing each set for 2 weeks, if I can change sets every 10 days or so.  That could make the total treatment period much shorter.  Would that be a good idea?  Please let me know. Thanks!”

 Dr. Pannu:

“It’s common to feel more tightness for the first two/three days whenever a person starts on the new aligners. However you can’t decrease the total time frame per aligner. Even though you don’t feel the pressure after few days of wear, it doesn’t mean that it is not working. You need to give proper time for the root to move as well along with the crown portion of the tooth.”

Are Invisaligners sharp on the gums?

“Invisalign aligners have minimal contact to the gums so you shouldn’t experience any gum irritation. Dental hygienists are keen on recommending this product. Your case belongs to the typical age group for Invisalign treatment. It’s never late to get started on Invisalign, as long as the periodontal (jaw bone) health allows.
